Office Policies/Appointments
Contact us at (480) 567-9771 to schedule an appointment
Insurance
We currently have providers in-network with Aetna, BCBS, UHC, and Medicare. Our biller, Laura, will need your insurance information prior to the first appointment, otherwise you will be responsible for the entire session fee.
Payment:
Payment is due when services are rendered. Credit cards, checks, and cash are accepted. If you pay by a check that is ever returned for insufficient funds, we will expect you to make good on the check and to pay me for any service charges levied by my bank.
Appointments
Appointments are booked directly with Dr. Anderson. Initial evaluations are typically one hour long. Individual sessions are typically 45-50 minutes long. Unlike other doctor visits, sessions begin and end promptly. Due to limited time slots, it is important to inform us if you need to cancel or reschedule an appointment. Our policy is to charge my full-fee for no shows and cancellations made within 24 hours of the schedule appointment time.
Lateness
The scheduling of an appointment involves the reservation of time specifically for you, and we will wait the entire session time for you to arrive. If you are late, we will meet for whatever amount of your time remains. However, you will still be required to pay for the full session.
Cancellations and No-Shows:
A minimum of 24 hours notice is required for rescheduling or cancellation of an appointment. If an appointment is missed or cancelled with less than 24-hours notice, you will be charged a $100 cancellation fee. Please note that it is illegal for us to bill the insurance for services not rendered. Therefore, you will be personally responsible for the fee. In the rare instance in which we both agree that there was an unexpected event or circumstance that could not be avoided, we will work together to reschedule the appointment.
