First Appointment - upon scheduling an appointment, you will be sent a link to an online client portal, Simple Practice.  There you can sign the privacy practices and informed consent forms as well as fill out intake forms and a credit card form for automated payments. 

IF YOU ARE A NEW CLIENT, YOUR APPOINTMENT WILL NOT BE CONFIRMED UNTIL YOUR INSURANCE INFORMATION AND CREDIT CARD INFORMATION IS PROVIDED.
