New Patient Forms
Please note that we have moved most of my client paperwork to an online portal, Simple Practice. Once scheduled for an appointment, you will receive an email with a link from Simple Practice in order to set up an account.
Once you set up an account on Simple Practice, you can sign the required privacy practices, informed consent, intake, and demographic/background information. There is a credit card form for automated payments and we require a credit card to be on file in case your appointment is not reimbursed by insurance or you cancel with less than 24 hours notice. We will never charge your credit card without notifying you first.
***We require 24 hours notice for cancellations and charge a $100 fee for cancellations not made prior to 24 hours notice.***
